We provide in-home support for caregivers and family members to help address the needs of the individual presenting challenging behaviors.

We value the individual we serve as a person first and foremost, with their own preferences for the support they are seeking. We incorporate feedback from caregivers and family members to create an intervention plan that can be reasonable and applicable to achieve goals.

Our team values individuality and recognizes that each individual we serve have specific needs and preferences. Each provider receives extensive training and will have prior experience working with individuals presenting challenging behaviors. Our licensed supervisors will ensure that appropriate clinical oversight of services are monitored and evaluated.

Independent living skills workshops at Spread Your Wings

The independent living skill workshops being offered are developed to provide direct training to adults so that they learn new and healthy independent living skills such as banking, budgeting, consumer advocacy, personal health care and many more.

Independent Living Skills Assessment

This evaluation tool is comprehensive, asset-based, and aims to highlight the consumer’s strengths and abilities. The assessment is based on consumer accomplishment of tasks through direct and indirect observation, self-assessment, and interviews. Upon completion of the assessment, a thorough summary of the evaluation along with recommendations is provided. The assessment is used only as a guide and may be altered depending on the individual needs of the client.

SELF-DETERMINATION PROGRAM


The Self-Determination Program (SDP) is designed to empower individuals with developmental disabilities by giving them greater control over the services and supports they receive. This program allows participants to tailor their care plans according to their specific needs, preferences, and goals, fostering independence and enhancing quality of life.

Key Features of the Self-Determination Program:

  • Person-Centered Planning: Each participant works with a support team to create a personalized plan that reflects their unique needs, preferences, and aspirations. This plan guides the selection of services and supports.
  • Individual Budget: Participants receive an individual budget, which they can use to purchase services and supports that align with their plan. This budget is based on the cost of traditional services that the participant would otherwise receive.
  • Flexibility and Choice: Participants have the freedom to choose their service providers, including hiring family members or friends, and can customize their services to better suit their lifestyle and goals.
  • Support Brokerage: Participants can work with a support broker or a financial management service (FMS) to help manage their budget and administrative tasks, ensuring they stay within budget while maximizing their resources.
  • Empowerment and Independence: By making their own decisions and having control over their supports, participants gain confidence and skills that promote independence and self-sufficiency.
  • Quality Assurance: Regular reviews and monitoring ensure that services meet the participant's needs and that funds are used appropriately. Participants are supported to make adjustments to their plans as needed.

The Self-Determination Program shifts the focus from a system-driven approach to a person-centered model, enabling individuals with developmental disabilities to lead more autonomous and fulfilling lives.

SUPPORTED LIVING


Supported living services (SLS) help individuals exercise meaningful choices and control in their daily lives, including where and with whom to live. SLS is designed to foster individuals’ nurturing relationships, full membership in the community, and work toward their long-range personal goals. Because these may be life-long concerns, Supported Living Services are offered for as long and as often as needed, with the flexibility required to meet a persons’ changing needs over time, and without regard solely to the level of disability. Typically, a supported living service agency works with the individual to establish and maintain a safe, stable, and independent life in his or her own home. But it is also possible for some individuals to supervise their services themselves, to secure the maximum possible level of personal independence.

At Spread Your Wings, our SLS program consists of a broad range of services to adults with developmental disabilities who, through the Individual Program Plan (IPP) process, choose to live in homes they themselves own or lease in the community. Our program will include:

  • Assistance with selecting and moving into a home.
  • Choosing personal attendants and housemates
  • Acquiring household furnishings
  • Common daily living activities and emergencies
  • Becoming a participating member in community life
  • Managing personal financial affairs, as well as other supports

Individuals who choose to live in their own homes, and their agencies or other people who support them, often will need information about affordable housing options, sources of financial support such as Supplementary Security Income (SSI), and how to stretch a limited budget to meet living expenses. These are the ordinary challenges that are inseparable from a truly self-directed life in the community. For the many adults for whom SLS makes great sense, such challenges are often also road signs on the path to a satisfying life.
